Included in this visit is a full day of consulting for the development and management of your facility!

This is a great opporunity to learn how Grand Island Physical Therapy and the M.O.G.SM have been industry leaders becoming an Integrated Health Promotion Service Company.

Visit us and experience a Medically Oriented Gym in operation. The professionals you will meet have been part of the development and operation of the M.O.G.SM since it opened in 2005. This team has strategically planned, and effedtively implemented a successful fitness center into a phyiscal therapy practice. You will have the opportunity to learn from their expertise as you begin the process of developing health promotion services into your site.

M.O.G.SM License Agreement

• Developmental Partners will be given phone support and the opportunity to participate in group activities only.

• Operational Partners are facilities that have begun incorporating M.O.G.SM programs into their clinics. They will have full use of the M.O.G.SM name and logo. They agree to abide by the M.O.G.SM standards and guidelines and maintain geographical ownership of the M.O.G.SM name.

3 Year Commitment to M.O.G.SM Co-op,

M.O.G.SM Co-Operative Agreement

• Club operating policies and procedures manual.

• Use of M.O.G.SM signature Be S.A.F.E Programs, including access to marketing materials.

• The sharing of M.O.G.SM Group member data and outcomes.

• Participation in the development and sharing of new M.O.G.SM programs.

• Quarterly group network conference call for program sharing and updates.

• Quarterly newsletter which includes the tracking and reporting of industry trends.

• 2 co-op group meetings per year.
